Requesting Electronics Recycling (SALVAGE)

Introduction

This article will explain how to fill out and submit a salvage form to the Data Center to recycle electronics equipment

Franklin County Fixed Asset Transfer Form (Required to be submitted to the Auditor’s office for Capital Assets)

The Data Center requires the Excel (.xlsx) version of the form to be submitted with the initial ticket in order to schedule a salvage pickup. A pdf version will be provided to sign for recordkeeping purposes.

 

  1. Please download the salvage form above

    1. Please Rename the Document using your agency name (or acronym), and the month/year that you are submitting the salvage request.

      1. ex: FCDC March22

  2. Complete the top portion of the salvage form

    1. Transferring Agency Name = Your agency name or agency acronym

      1. ex. Data Center or FCDC

    2. Contact Person (printed Name) = Your Name, or the contact person for the salvage request.

    3. Authorized Signature = Leave this blank, this field gets signed once salvage is picked up by the Data Center.

      1. If your agency requires a signature from someone that is not available at the time of the salvage pickup, you may have them pre-sign a pdf copy of the form, and submit along with the excel copy, but they will need to be prepared to sign a new form if there are any discrepancies found on the items when Data Center is picking the equipment up.

    4. Phone = Phone number of the above contact person

    5. Receiving Agency Name section = Leave this blank, the Data Center representative will fill it out prior to picking up your salvage.

  3. Enter the items to be salvaged

    1. Description = a description of the device to be salvaged

      1. ex. Desktop, Laptop, All In One Computer, ThinClient, Server, monitor, Printer, Phone, etc (please do not use CPU, Tower or Computer in these fields). If you have mice/keyboards/cables or other generic untrackable equipment to salvage, these may be entered on a single line.

    2. Model = Manufacturer’s Model Name of the device

      1. ex. Optiplex, Latitude, Yoga, LaserJet, iPhone, iPad, EliteDesk

    3. Model Number = Manufacturer’s Model # of the device

      1. ex. 7050, 5050, TB16, 800 G2, P3015

    4. Serial Number = Service Tag (preferred for Dell), or Serial number of the device being salvaged

    5. Asset Tag # = Asset tag of the device if your agency has one.

    6. Comments = any additional comments that may need to be added to the equipment in question

    7. Agency = Your Agency Acronym. If you are not sure, you may leave this blank and the Data Center representative will fill it out for you.

    8. Manufacturer = Manufacturer of the device

      1. ex. Dell, HP, etc.

    9. HDD Removed = If the device is a desktop or laptop, or other device containing an SSD/HDD, this field must be filled out . Type Yes if your agency removed the SSD or HDD from the device, or No, if it still needs to be removed.

    10. Mobile/Network Device Wiped = If the device is a mobile device, network device, or other device with data storage, such as a cell phone, tablet, or other device where the hdd/ssd is inaccessible, type Yes if your agency wiped the device, or No if it still needs to be wiped. The data center requests that all mobile devices and network devices be factory reset prior to receiving the equipment as salvage.

  4. Feel free to add or remove row’s from the spreadsheet if you need more/less space than what is provided.

  5. Submit the completed salvage form to the Data Center by one of the following methods.

    1. Recommended: Submit a ticket directly in the Franklin County Enterprise Support Portal: https://franklincountyohio.atlassian.net/servicedesk/customer/portal/1/group/1/create/20

    2. Send an e-mail to helpdesk@franklincountyohio.gov

 

If you have information that is missing on the form, leave the field blank, DO NOT enter N/A, or None. It will be assumed that the information was not able to be obtained from the device.

What to Expect

  1. A Helpdesk employee will reach out to the submitter of the ticket, to schedule a time to process and pick up/drop off the salvage.

  2. After the salvage has been cross checked to the salvage list, the data center employee will sign a digital copy of the salvage form, and then will send the form to the initial submitter to sign and send back.
